Intermountain Healthcare, a Story Health partner on heart failure care

Intermountain Healthcare and Virtual Specialty Care Co Health story announced a strategic partnership to allow the health system’s heart failure patients to access care outside of the clinic.

Calif.-based Story Health offers telehealth., health coaches, AI and connected devices to treat heart failure patients at home. It also allows doctors to access patient data and get personalized recommendations based on a doctor’s historical prescriptions.

As part of the partnership, Intermountain clinicians and health tech company coaches will collaborate to provide patients with personalized treatment and monitor medication access and adherence between visits.

Coaches will act as a go-to between patients and their providers, monitoring patient progress and answering questions via text, email or call.

“Patients have a long journey with their health, and an important part of that journey is after they leave the clinician’s office,” said Tom Stance, CEO and cofounder of StoryHealth, in a statement. “Our solution combines the latest AI-integrated electronic health record technology with real health coaches to directly identify and remove the most disruptive barriers to the patient journey. StoryHealth patients is excited to partner with Intermountain to transform the continuum of care.”

A big trend
